Clinical Significance: | The widespread use of tricyclic antidepressants to treat depression has been accompanied by an increased incidence of tricyclic antidepressant overdoses. An estimated 5,000 to 10,000 cases of tricyclic antidepressant poisoning occur each year in the United States. Differentiating the patient who has taken an overdose of tricyclic antidepressants from one who has ingested hypnotics or sedatives is important due to the life-threatening complications associated with many acute overdoses of tricyclics. Among the serious complications that can occur are grand mal seizures, cardiac failure, and arrhythmias. The identification of tricyclic antidepressants as the overdose drug helps in selecting the appropriate treatments. Methods historically used for detecting tricyclic antidepressants in serum include gas-liquid chromatography,4 high-performance liquid chromatography, and radioimmunoassay. The Emit Tricyclic Antidepressants Assay detects the presence of a class of drugs and their metabolites. A positive result should be confirmed by an alternative method. While confirmation techniques other than GC/MS may be adequate for some drugs of abuse, GC/MS is generally accepted as a rigorous confirmation technique for all drugs, since it provides the best level of confidence in the result |
